Reason for referral: Patient presents with persistent frontal headache (3 days) and known hypertension with elevated BP (152/94). Fundoscopy performed at clinic level shows no papilledema, however referral for comprehensive eye examination to rule out hypertensive retinopathy and assess visual acuity. Patient also reports photophobia as associated symptom.
Reason for referral: Suboptimal glycemic control with HbA1c at 7.2% (target <7.0%). Fasting glucose elevated at 142 mg/dL. Patient is currently on Metformin 500mg BID. Requires comprehensive diabetic review including medication optimization, dietary counselling, foot examination, and annual screening labs. Patient also has comorbid hypertension requiring coordinated management.
